What Do They Do? The Senior/ Community Center can host groups from 16 to 500. The flexibility of our meeting space assures you of our ability to respond to the meeting requirements of your group at reasonable rental rates. The new 10,000 square foot Senior/ Community Center raises the standard for meetings with the ultimate in technology, sophistication and service. Our state-of-the art facility features up to four conference rooms with flexible configurations. Our commercial kitchen is equipped with ovens, refrigerators, freezer, ice machine, cookware and serving utensils. The City of Clearlake is pleased to say that the Senior/Community Service Center, offers 5,000 square feet of actual meeting space. Our meeting room arrangement can accommodate 335 people in our Multi-Purpose Room for classroom type seminars and the rooms range downward to 25 people in our small, intimate meeting rooms. The Senior/Community Service Center is an ideal meeting place with exceptional facilities that will make your next meeting comfortable and convenient. The Senior/Community Service Center is ideal for club meetings, weddings, industry and professional meetings, product demonstrations or employee training events.

What Do They Do? Line Dance, Low-impact exercise class, Bingo, card playing, Monthly dances Monthly breakfasts: (3rd Sunday of the month 8:00 a.m. to 12 noon) They have a Nutrition Program, Outreach information and offer congregate meals Monday through Friday at 12:00 noon. Home-delivered meals are provided for frail, home-bound persons over the age of 60. We distribute Brown Bag to seniors. Whether on site or delivered, there is a suggested donation of $3.00 each. Other services offered are: AARP (income tax preparation), Podiatrist, Hearing Test, Blood Pressure checks & Diabetic Support Group, Sewing Club, 55 Alive. CPR, First Aid, and Art classes. What Do They Do? MSSP provides long term case management services to help frail elderly persons prevent premature, unnecessay institutionalization. Case management services include: A Case Manager makes client contact at least once a month. Some months, several visits are required An RN/Health Practitioner visits at least quarterly for health assessment and referral. Some months many visits are required to assess, educate, coordinate, and refer. NOTE: The MSSP RN/Health Practitioner does not provide skilled nursing care. The RN/Health Practitioner or Case Manager coordinates with physicians, discharge planners, home health agencies, and other health care/social services providers. A common health service MSSP provides is weekly Vital Signs Monitoring and Medication Setups. We contract with an LVN from a home health agency. MSSP may also: Arrange physicians' and other appointments. Provide escort/transportation to medical and other appointments. Interpret and reinforce physicians' and health providers' instructions Arrange incontinence supplies. Advocate for benefits and services. Provide information and referral re: eldercare and resources. Assist with Long Term Care Planning. Provide home safety assessment and arrange for adaptive equipment. MSSP has a small budget to, when appropriate, purchase some temporary, short term services: Family respite: Emergency, short term chore work and/or personal care. Nutritional consult and food supplements. Minor home repairs. Supportive and/or Therapeutic Counseling Communicall or ER!!! NOTE: Medicare and Medi-Cal benefits must be exhausted prior to MSSP purchasing services

Our Program offers comprehensive services and support to residents of Lake County and their families facing terminal or life threatening illnesses. Services include as ordered by MD: skilled nursing including pain and symptom management, home IV therapy and patient/family education The Hospice Team also includes home health aids, social workers, spiritual and bereavement counseling. All services provided at no charge to the patients' family. Medicare, MediCal and most private insurance carriers provide coverage for hospice care.We serve clients of all ages from terminally ill children to the elderly. Our County Wide Bereavement Program is available to any resident who has lost a loved one to death. Our specialized bereavement groups include, but are not limited to, sudden death, suicide, and our "Wings of Hope Program" specially developed for children, teens and parents.
